One common concern before IVF is whether children born from IVF/ICSI have higher autism risk. Media and social media misinformation causes anxiety. Here's what the science says.
Bottom Line
When confounders are properly controlled, IVF/ICSI does not significantly increase autism risk. Earlier apparent risks came from underlying parental factors, not from IVF itself.
Key Studies
- Sandin S et al. JAMA 2013 — Swedish study of 2.5 million children. IVF alone does not increase autism. ICSI in severe male factor may have small increase.
- Kissin DM et al. 2015 — US study, after controlling for maternal age, singleton vs multiple, and baseline factors, no difference from natural conception.
- Bay B et al. 2019 — Meta-analysis confirmed apparent risk comes from multiple gestations, not IVF technique.
Real Autism Risk Factors
- Advanced parental age (especially paternal > 40)
- Multiple gestations
- Preterm birth and low birth weight
- Family history of autism
- Parental genetic factors
How to Minimize Risk
- Elective Single Embryo Transfer (eSET) — reduces twin risk
- Complete IVF before advanced age
- PGT-A when indicated
- Optimal prenatal care to prevent preterm birth
IVF/ICSI is not a cause of autism. Couples at higher baseline risk can discuss additional risk-reduction strategies with their doctor.
