Single Dominant Follicle in IVF

During ovarian stimulation for IVF, sometimes only one follicle becomes dominant while others lag behind. This creates a decision point for patient and physician.

Why It Happens

Management Options

1. Delayed Trigger + Increased FSH

Wait for other follicles to catch up. Add LH activity (Menopur, hMG).

2. Convert to IUI

If multiple follicles unlikely, switch plan to IUI to salvage the cycle at lower cost.

3. Continue and Retrieve Anyway

Retrieve one egg, proceed with IVF/ICSI. Fewer embryos but pregnancy still possible. Reasonable for older patients or previous cycle failure.

4. Cancel and Restart

Cancel current cycle, adjust protocol next cycle: try DuoStim, Random-start, or DHEA/CoQ10 priming.

Random-start Protocol

For patients who repeatedly develop single dominant follicles, Random-start ovarian stimulation (starting stimulation on any day of the cycle) may better capture the follicular wave.

DuoStim (Double Stimulation)

Two stimulation rounds in one cycle — first in follicular phase, second in luteal phase after first retrieval. Maximizes egg yield in short time — good for poor responders.
